The traditional approach to the diagnosis and treatment of illness as practiced by physicians in the Western world since the time of Koch and Pasteur. The physician focuses on the defect or dysfunction, within the patient, using a problem-solving approach.

Robert Koch, a distinct medical officer in 1870, discovered Anthrax to be a deadly disease of cattle. And Louis Pasteur, a French chemist discovered a wellness model to kill the microorganisms that caused Anthrax.

Pasteur’s work with silkworm parasites and air-bourne germs led him to propose the germ theory of disease. He pressed doctors to disinfect their instruments by boiling and steaming. Pasteur discovered that septicemia was caused by an anaerobic bacterium. Therefore, he urged surgeons to use clean instruments, wash their hands, and disinfect their gauze and bandages.. Pasteur heated Anthrax germs and inoculated 25 sheep. All 25 sheep survived, demonstrating that the weakened Anthrax lost its virulence but still could confer immunity.

The wellness model is a multidimensional state of being describing the existence of positive health in an individual as exemplified by the quality life and a sense of well-being. Wellness is an active process of becoming aware of and making choices toward a more successful existence.

The medical model states that the path to being “fixed” comes from sources outside of us via remedies and treatments that were performed by clinicians upon us. The best that one could hope for is to have our “illness” be controlled, or go into remission, to be maintained rather than getting better or non-existence. However, the Wellness Model takes recovery model one step further. Every action counts. Nothing is insignificant.

Industrialization affected medicine in many ways. Such as communications that developed, allowing ideas to be exchanged more quickly. These changes caused doctor’s to move away from pre-industrial ideas and look for scientific reasons. As with the case of Anthrax, Koch looked for treatment while Pasteur searched for scientific reasons.

Medicine during the post-industrial period. During the 19th century medicine made great advances. Louis Pasteur proved that microscopic organisms caused disease. And once doctors knew what caused diseases they made rapid headway in finding cures or preventions rather than treatment only.

One of the key challenges facing and administrative health care managers  is matching demand for services with resources. Not only do the right numbers and types of staff need to be hired, but staff members must be scheduled in way that is efficient yet maintains patient safety and other quality outcomes.

Infant Male Circumcision

Routine Infant circumcision (RIC) is the most common surgical procedure performed in the U.S. It is a very common misconception that there are many health benefits to infant male circumcision. However, no national medical organization in the world recommends RIC. Infant male circumcision in the U.S. started in the late 1800s. Dr. John Harvey Kellogg (yes, the man who created cornflakes) is who is most notably known for promoting and normalizing RIC as a means to cure masturbation, and as a way to punish people for masturbating. More outrageous claims were made, like that circumcision prevented epilepsy, insanity, schizophrenia and blindness. RIC took off from there, and as time went on, more and more reasons were found to circumcise infant males at birth. The most common reasons you hear for circumcising infants at birth is a reduction in HIV, and to prevent sexually transmitted diseases, and other infections. However, medical literature shows that it is actually the opposite, and circumcision can increase the risk for certain health issues. There are also immediate complications of circumcision such as hemorrhage, infection, surgical mishap and death. In this paper, I will use my ten sources, along with my own personal knowledge to go further into detail on how RIC is not only unnecessary, but that it also causes more harm than good.
The number one reason that people chose to circumcise their newborn babies is because they are under a false assumption that circumcision greatly reduces the risks of HIV, STDs, and other infections; It’s assumed that when a male is intact, that he must be dirty. However this is just not true, infections in intact males do happen, just as females can get infections, but most of the infections such as UTI’s or yeast infections are due to medical misinformation. It is a common thought that when a male is intact, he has to pull the foreskin back to make sure everything is clean, but this is exactly what can cause infections. The penis is self-cleansing just as a vagina is; all you need to do is simply wipe it like a finger with nothing more than water, no soap needed. It is also very painful, the foreskin will naturally retract on its own; the foreskin serves as a protective barrier to the glans (head) of the penis the same as your fingernail protects your nail beds. Forcibly retracting the foreskin to clean irritates the penis, and opens it up to infections. However, it’s also good to note that UTI’s and yeast infections are very easily treated with antibiotics, and that females get more UTIs than men. We would not circumcise a female to protect against infections, so why would we circumcise a male to protect against infections? It is also a common thought that circumcision reduces the risks of STDs and HIV; however, it is actually the opposite, and being circumcised results in more cases of STDs and infections. In the USA, during the AIDS epidemic of the 1980s and 90s, about 85% of adult men were circumcised (much higher rates of circumcision than in Africa), and yet HIV still spread. There is nothing dirty about the normal genitalia of men; every part of our body serves a purpose.
Infant circumcision also does not come without complications. An infant’s foreskin has more than 240 feet of nerves, 20,000 nerve endings, and 3 feet of veins, arteries, and capillaries – circumcision removes all of them, causing the infant tremendous pain. Immediate complications following RIC include botched circumcisions, where the circumcision is performed incorrectly, or too much foreskin is taking off, leading to painful erections later in life, or in some cases amputation of all, or part of the penis. Other complications include death; infants do not have much blood in their bodies as is, if the circumcision is performed incorrectly, and the infant loses too much blood, death can be a result. It is also unknown right at birth if an infant has any type of bleeding disorder. The long-term side effects of RIC include emotion, sexual, and female sexuality effects. PTSD is something we normally associate with military members, but they are not the only ones who can get PTDS. Memory starts from birth, just because someone doesn’t remember something, does not mean that memory had no effect on them psychologically. PTSD is a normal response to an abnormal event. Neonatal genital cutting is an event in which a newborn infant experiences extreme levels of pain, terror, and helplessness, so it fulfills the criteria for PTSD. Circumcision removes more than 50 percent of the normal skin and mucosa from the penis. This skin and mucosa is provided by nature to allow for the expansion of the penis during erection. Circumcision can result in painful erections later in life due to the foreskin being removed. The foreskin also plays a huge role in sexuality; the foreskin provides a natural gliding action that helps with lubrication, when the foreskin is removed, the penis actually pulls lubrications out of the vagina, resulting in drier sex. The foreskin is the most erogenous spot on the penis, it contains up to 20,000 nerve endings; circumcision removes most, if not all of these nerve endings. For an accurate comparison, the female clitoris only has up to 7,000 nerve endings. The glans, commonly known as the “head” of the penis is meant to be an internal organ, so when an infant is circumcised, overtime the glans will become calloused, making sex less pleasurable. This is by no means saying that sexual intercourse is still not enjoyable by men, however, without their foreskins, men are missing out on more pleasure than they already do get.
The foreskin is an erogenous sexual organ that serves many functions; it is not just a “flap of skin” as some would say. It is estimated that the foreskin has upwards of twenty functions, some of the most important ones being immunological functions. Just as the eyelid protects the eye, the foreskin protects the glans, keeping its surface soft, moist, warm and sensitive. It also maintains optimal warmth, pH balance, and cleanliness. Just like the vagina, the foreskin is a self-cleansing sexual organ. The urinary meatus (the opening of the glans through which urine and semen flow), is an entry point into the body. From infancy to adulthood, the foreskin protects the glans and urinary meatus from contaminants of all kinds. During childhood, the foreskin is also usually firmly attached to the glans to prevent contaminants from getting into the urethra. The neck of the foreskin places the vulnerable urinary meatus at a distance from the external environment and defends it against contaminants. The fusion of the foreskin and glans and the nonexpandability of the foreskin in the child’s penis are therefore necessary for the health of the child. Even after the foreskin separates from the glans and becomes retractable, which is usually around puberty, it continues throughout life to cover the glans and meatus in order to protect from dirt, contamination, abrasion, or bacterial invasion. The mucous membranes that line all body orifices are the body’s first line of immunological defense. Glands in the foreskin produce antibacterial and antiviral proteins such as lysozyme. Lysozyme is also found in tears and mother’s milk. Specialized epithelial Langerhans cells are found in the foreskin’s outer surface. Plasma cells in the foreskin’s mucosal lining secrete immunoglobulin’s, antibodies that defend against infection. Rigorously controlled studies have also demonstrated that the foreskin plays a protective role in shielding the rest of the penis and thus the rest of the body from the contagion of common sexually transmitted diseases encountered during sexual activity. To help fight harmful bacteria, the foreskin has a rich flora of beneficial bacteria. Friendly bacteria exist within the body and are found on all body surfaces and throughout the gastrointestinal, genitourinary tract (the urinary system that runs from the kidneys, through the bladder, and out the penis), and the mouth. Friendly bacteria also thrive in the eyes. Without the presence of friendly bacteria, the human body would be vulnerable to attack from all kinds of bacteria.
The good bacteria that live in the inside of the foreskin are similar to the bacteria found in the mouth, nose, the female genitals, and the skin in general. This good bacteria is both harmless and highly beneficial; without this friendly bacteria, the urethra would become an easy entry point for germs and harmful stains of bacteria, which could cause disease.
Circumcision is not as prevalent as one might think; 85% of the world is not circumcised. So surely, if not being circumcised can lead to so many problems, that number would be much, much lower. The United States Centers for Disease Control and Prevention (CDC) released new statistics at the International AIDS 2010 conference in Vienna, Austria. The new statistics showed a steep drop in the number of circumcisions performed in the United States. The CDC data showed that the incidence of circumcision declined from 56 percent in 2006 to 32.5 percent in 2009. According to these statistics, non-circumcision or genital integrity has become the normal condition among newborn boys in the United States. In the Muslim World, circumcision is almost universal, as well as parts of South East Asia, and parts of Africa. However, everywhere else in the world the rates are either very low, or completely non-existent unless medically necessary. In Europe, circumcision rates are so low, that it is seen as a horrible, barbaric surgery to have done, and even suggesting it to someone who is from Europe will be sure to get you a lot of strange looks. It really says a lot that besides America, no other first world country practices circumcision, and the main reason that those countries due circumcise are because of religion.
Many infants are circumcised under the guise of religion, when in reality the only two religions that believe in circumcision as a necessary custom are Judaism and Islam. For the Jewish, circumcision is a commandment from God (in the Old Testament), which is a sacrifice of flesh to honor the covenant between God and the chosen people. Jewish boys are circumcised 8 days after birth. For Muslims, circumcision is a cultural tradition that recognizes them as descendants of Abraham, and also a rite of passage from childhood to manhood. Muslim boys usually get circumcised sometime before puberty. However, Christianity, and any other religion that follows The New Testament, is forbidden to circumcise. The bible (New Testament) refers to circumcision as mutilation and clearly says that any Christian who practices circumcision has put no faith in Jesus Christ for salvation. It is also important to note, that circumcising an infant because it’s your religion, is violating that babies freedom of religion. That baby may grow up to practice another religion, or they may grow up and be an atheist, the choice to make religious decisions regarding someone’s body should be left up to the person who owns the body.
Routine Infant Circumcision is unconstitutional on the basis of the Equal Protection clause of the 14th Amendment. Female circumcision was made illegal in 1996; therefore, it is unconstitutional to not extend the same courtesy to males born from that time on. Many will say that female circumcision is not the same as male circumcision, but female circumcision is very comparable to male circumcision. There are four types of female circumcision, most people who think of female circumcision only think of the fourth type, where the entire vagina is sewn shut. However, that is the least common type, the most common type being just a prick to the clitoris, and the second most common type being just the tip of the clitoris being cut off. The reasoning for female circumcision is very similar to male circumcision. It is said to be more hygienic, and reduce disease. Also, the way female circumcision is viewed where the most common types are performed, is the same way Americans view it. They say it is just the normal thing to do, it is cleaner, everyone does it, and it is weird not to do it. RIC also goes against a doctors Hippocratic Oath, which states, “First, do no harm” circumcising a newborn male, for no medically indicated reason is harming that infant. Unnecessarily invasive procedures should not be used where alternative, less invasive techniques are equally efficient and available. It is important that doctors keep up to date and ensure that any decisions to undertake an invasive procedure are based on the best available evidence. Therefore, to circumcise for therapeutic reasons where medical research has shown other techniques to be at least as effective and less invasive would be unethical and unnecessary. Unless the circumcision is medically necessary, performing it is purely cosmetic. It is a violation of basic human rights; no one should have parts of their bodies amputated off without their consent unless medically necessary. If a male wants to be circumcised later in life, which is doubtful, then that decision should be his, and his only. Everyone, be they male or female, has the right to bodily integrity.
Routine infant circumcision is a major problem in society today. It is by definition, male genital mutilation; what are we teaching our boys when at only a few hours old, we amputate a perfectly healthy, normal sexual organ with many functions? Many reasons are given to circumcise, be they for health reasons, religion, or culture, as is seen in America. But throughout this literature review, many reasons have been provided showing the adverse effects circumcision can have, immediately, and further on in a males life. It is time that America breaks out of its bubble, and explores what the rest of the developed nations are doing. No medical organization in the world recommends circumcision, and in Europe it is heavily frowned upon and finding a doctor to even do it without medical cause would be very difficult.

Implementing Six Sigma at Wishmewell Hospital

Emergency departments (ED) at many hospitals have been overwhelmed in the past year, as more patients without health insurance use the ED as a primary care solution. Wait times in the ED are increasing as more and more Americans are using the Emergency Department for their general health concerns. Across the country, the average ED wait time is now 222 minutes (approximately 3 hours, 42 minutes).

Wishmewell Hospital’s average wait time in the ED is more than five hours, and the board of directors is concerned about this long wait time. The danger is that a patient’s condition may escalate during his/her waiting time in Wishmewell’s ED.

Rationing issues in healthcare

The concept of rationing healthcare is dealt with on a daily basis. Healthcare rationing in the United States exists in various forms. Access to private health insurance is rationed based on price and ability to pay. For this assignment, using the situation given below, apply the S.W.O.T. Situational Planning Strategy which was described in Chapters 4 and 7 of your course text:

Situation:  You are the C.E.O. of Community South Medical Center, a large, urban for-profit healthcare facility.  This institution has comprehensive health services including acute care, residential care, independent living, in-home nursing, hospice, neonatal, and pediatric services, advanced cardiac services, a major trauma center, a center of excellence for pulmonary services, and a neurosurgery center.

Community South Medical Center has a positive bottom line and is striving to enhance health services for the population it serves.  The patient care mix has shifted in the last three years and its reimbursement source is currently:  25% Medicaid (up 20%); 35% Medicare (down 12 %); 25% employer sponsored health insurance (unchanged), 8% managed care (down 20 percent), 4% private pay (unchanged) and 4% no-pay (up 25%).

The Medical Center is in an older urban area.  Businesses with well-paying jobs have gradually been replaced by smaller shops and other small businesses.  Overall, the community is trending towards a predominantly elderly population.

The Medical Center has an excellent reputation and has been recognized in the past by receiving a Baldrige Center of Excellence evaluation and a The Joint Commission (T.J.C.) approval for their quality of services; however, recent self-inspections indicated a slight decline in compliance with T.J.C. standards.  Patient satisfaction survey results have an overall mean of 95 percent.  The medical staff strongly supports new program development and there is an abundant supply of physicians.

Currently, the major issues confronting the Community South Medical Center are: an identified shortage of clinical staff including registered nurses, respiratory therapists, and medical technologists; non-interfacing information technology systems; antiquated facilities and infrastructure; decline in T.J.C. compliance on internal audits; and a shift to its financial mix.

Action:  As the C.E.O., it is time to embark on the annual updating of the organization’s strategic plan.  The last major revision to the strategic direction was completed three years ago.

Epidemiological Study Proposal

Introduction

Male sexual assaults in the military are significantly understudied. With the current movement within each of the military branches of the United States military it is important to recognize and understand male sexual assaults in the military. The United States Marine Corps, The United States Air Force, The United States Navy, and the United States Coast Guard all have a program within their branches called the Sexual Assault Prevention and Response otherwise known as SAPR. The United States Army is the only one that differs from the branches and has a program called the Sexual Harassment/Assault Response Prevention Program or better known as SHARP. The Department of Defense requires all branches of the service under what is known as Title X to have a specific manner to responding to sexual harassments and sexual assaults on military members and their family members over the age of 18 as well as civilians and their family members over the age of 18 when they are stationed in an overseas environment.

The difference between the two programs lies within sexual harassment. Although each branch must also have standard operating procedures for sexual harassment the SAPR program leaves the sexual harassment section to the Equal Opportunity section of the military. The United States Army’s program, SHARP, holds harassment and sexual assaults together under one response and prevention effort. The idea behind the United States Army’s approach is to combating sexual assault one must look at sexual harassment. In many cases sexual assaults are preceded by sexual harassment. By taking ownerships of each part the United States Army has been able to develop a prevention and response method that starts with recognizing sexual harassment and stopping it at this level, which they consider the lowest level. All branches of the military have been forced to make a movement at confronting sexual harassment and sexual assault within the military. They are making great movement towards the ideas of bystander intervention, educating the military and civilian force on what sexual harassment and sexual assault looks like, and their responses to victims of sexual harassment and sexual assault. However, the prevention and response initiatives thus far still has a very strong focus on female sexual harassment and assault. This focus on female prevention and response decreases the trust that males have within the United States military to report sexual harassment and sexual assault. It helps perpetuate the myths surround male sexual harassment or assault such as males can’t be raped, male on males sexual assault is only committed by homosexual males, and other myths that are significant barriers to reporting and ultimately care for male military members.

In the February 2015 United States Army SHARP Summit held by General Raymond T. Odierno, the United States Army 38th Chief of Staff, stated that the top two issues that need significant attention is male on male sexual assault and retaliation. Although when looking at the data on sexual assaults the percentages reflect that females have the highest sexual assaults, males should not be ignored. Raw data shows that more males are being sexual assaulted than females. In 2014 Fiscal Year annual report to the President of the United States 5,300 reports of sexual assaults were filed by males (TRADOC, 2015). This is only the reported number and it is likely that the number is significantly higher due to the stigma of reporting for males. The United States Army even stated that they do not believe they fully understand the scope of the problem of male on male sexual assaults and they want to make it a priority to understand the scope.

Topic

Studying male sexual assault incidents among male U.S. military members is important because although the incidence of sexual assault on women in the military are statically higher the raw data indicates that more males are sexually assaulted than women in the military. It is important to understand not only the prevalence of sexual assaults on males, but also the incidents. How males are sexually assaulted speaks to if our current resources are applicable and meeting the needs of male sexual assault survivors and if current education is encompassing male sexual assaults appropriately. It has an added layer of importance due to the under reporting of male sexual assault.

Studying male sexual assault in the U.S. military in regards to access to the population is quite easy if the study is done within the confines of the Department of Defense. The Department of Defense already has unlimited access to the military population, access to military sexual assault data base, experts in the public health profession, yearly budget for sexual assault, and the time allotted within their current organization to construct and execute the research. The U.S. military has been identified as having a problem with sexual assault and have been tasked with decreasing and eliminating sexual assault within the military. However, what Congress has done by requiring them to get after sexual assault is to really address the problem of sexual assault throughout the entire United States. It is often said that the U.S. military is a microcosm of the population. If this is the case, a study done on male military members in regards to sexual assault could benefit not only the U.S. military, but also the United States in general. With the understanding of male military sexual assault incidents it could give more than just the Department of Defense a stepping-stone in combating sexual assaults against males. It could lend assistance to the entire country and their approach to male sexual assault.

For any research there are ethics that guide individuals and this topic is not an exception. Even though the study would be done within the Department of Defense there are still guidelines that need to be followed. Some of these are always minimizing risks and protecting the welfare of research participants, protecting confidentiality and privacy, obtaining informed consent of participants, submitting proposed studies for review, avoiding conflict of interest and partiality, assuring ethical requirements to colleagues and sponsors, and upholding all obligations to communities.

The impact of a study around male sexual assaults in the military is significant economically, socially, and politically. First economically, understanding an understudied social problem such as male sexual assault will provide a clearer picture of the problem so that prevention and resources can target the specific areas. By increasing the understanding, prevention, and resources for male military sexual assault victims there will be less impact on government health care, law enforcement, less money allocated to sustain reactive resources, and less negative impact on military missions. Socially understanding male military sexual assault will help bring the incidents of male sexual assault out of the shadows and into the light. By addressing it as a problem can bring about acceptance and understanding thus increasing reporting, and helping prevention and resources target a specific population significantly better. Finally, political factors include by increasing the awareness of male military sexual assault the U.S. military and Department of Defense are setting a precedent for policy and judicial changes throughout an entire country.

It has an added layer of importance due to the under reporting of male sexual assault. Due to all of these issues a study must be designed in order to address male sexual assaults in the military. To do this one must outline a specific study.

To start we need to understand that this study is on male sexual assaults in the military. It is to understand the number of male sexual assaults in regards to reported and unreported sexual assaults, barriers to reporting, how males are sexually assaulted, if prevention efforts are affective, and if resources are available to male sexual assault victims. This study will be an observational cross-sectional study. This was chosen due to the fact that the information that is being sought is gained from naturally occurring status, behaviors, attitudes, and other characteristics of males in the military. This study will be a one time interaction that will capture a snap shot of the current situation regarding male sexual assaults in the military.

The participants in this study will all be current United States active duty male individuals. These male participants will have no other criteria for selection except that they must be on current active duty and be male. This study will be done with the assistance of the United States Department of Defense. Due to this partnership, data on the males will come from the Department of Defense’s Defense Manpower Center, which holds all current information on military members. Currently there are approximately 1,226,000 males on active duty in the United States of America. To get information on male sexual assaults in the military a survey will be made available making sure to keep the vocabulary appropriate, use of simple sentence structure, and with little to no ambiguity and/or vagueness. The survey will go out to a random sample of active duty military members stationed throughout the world through the global email system directing them to a website where the survey will be available.

Of these 1,226,000 males on active duty a random sampling will be taken. Random meaning that everyone has equal chance of being selected. To get the random sample number we will take the total number of males on active duty service in the United States military and accept a 4% margin of error. This puts the amount of male active duty service members being surveyed at 1, 036 at a 99% confidence. However, as in most surveys not all who receive the survey will take the survey. With this in mind we will calculate for those who fill out the survey at 60% making the amount of male active duty service members needing surveys at 1,727. Once the information is gathered it is time to analyze the data.

The data will be analyzed by using Epi Info. Epi Info is simple in nature, but allows for rapid creation of data analysis even for those who are not very familiar with analyzing data. It can be imported from other data formats and Epi Info will also help with the reporting of the data. It allows for statistics, tables, graphs, and maps to be produced in a very simple manner thus conveying the results in a simple manner for a variety of individuals to understand. This is necessary in order to convey the results to a diverse group.

It is imperative that a study be designed and carried out in a manner that represents true data and answers the questions that are laid out. It is important to make sure that random sampling is done correctly in order to provide a true representation of the population. Ethical considerations in research as it pertains to humans are a significant aspect that must be considered when undertaking any new study. It was during the latter part of the 20th century when significant attention was give to the abuse of human subjects and finally to a development of a system for overseeing human research and framework for the ethical conduct in research involving humans (Iltis, 2008). Due to this, the code of Federal Regulations governs a large portion of the research conducted in the United States, but is not entirely the only overseer. The regulations require ethical insight that needs interpretation and has to be applied correctly and this is why the Office of Human Research Protections (OHRP), which is under the U.S. Department of Health and Human Service, can also offer additional guidance. Professional organizations and institutions also can and have produced codes of ethics or even guidelines that address ethical conduct of research on humans.

Clinical research allows the development of generalizable knowledge that does improve human health or increases the understanding of human biology (National Institute of Health, 2015). Those that participate in research make this all possible and must be treated with the upmost care and respect. Ethical guidelines are set to protect patient volunteers and also to preserve the integrity of the science. Some of the most influential codes of ethics are the Nuremberg Code, Declaration of Helsinki, Belmont Report, CIOMS, and the U.S. Common Rule. From these codes seven principles can be observed. They are social and clinical value, scientific validity, fair subject selection, favorable risk benefit ratio, independent review, informed consent, and respect for potential and enrolled subjects. It is these seven principles that must be observed in regards to ethics. Social and clinical value covers the idea that the research question should be important enough to justify asking subjects to accept some sort of risk or even inconvenience. Scientific validity assures one that the research will be designed in a manner that will get an answer that is understandable and valuable. Fair subject selection relates to the primary basis for recruiting and selecting subjects. Favorable risk benefit ratio relates to the uncertainty of the risk and benefits of the research and the minimization of the risks and maximization of the benefits. Independent review is meant to minimize any conflict of interest by having an independent review panel with no vested interest review the study to determine that it is ethically acceptable. Informed consent related to subjects being able to make their own decision on participation or even continuing in the research. Finally, respect for potential and enrolled subjects directly relates to how individuals should be treated before, during, and after the research (National Institute of Health, 2015).

Epidemiological research and regulations governing human research is no different than stated above. However, research in epidemiology can raise some unique ethical concerns. This point is crucial when looking at privacy, confidentiality, informed consent, study design, cultural sensitivity, reporting of results, blurring of the line between advocacy and scientific research, and possible conflicts of interest among individuals who fund the project and their specific results (Iltis, 2008). Many of these concerns are relevant for the study of United States Military Male Sexual Assault Incidents.

In research regarding male military sexual assaults concerns about privacy, confidentiality, lack of consent, and cultural sensitivity are primary ethical concerns. The addresses and names for the survey will be pulled from existing government databases. This might convey to some a lack of privacy and a lack of consent, but this is mitigated. Due to the fact that it is the U.S. Department of Defense (DoD) is working on the study the military members personal information in not confidential to the DoD. The survey will be done via Internet so responses will not have any personal identifying information on them. This mitigates the privacy and confidentiality issue of each subject. Lack of consent is mitigated by the fact that there is nothing in place to force compliance of the survey. The subject can determine if they want to take the survey or if they do not want to take the survey. All information obtained will be kept in the upmost secure manner and used only in this specific research. All information is given voluntarily. This is important due to cultural sensitivity. Military sexual assaults are not spoken of often in the military environments. There are stigma’s attached to male sexual assaults in America and especially male sexual assaults in the U.S. military. This concern is again mitigated by the fact that the survey is accessible via the Internet. However, consideration for how questions are developed and displayed is important and will be considered.

Although research is an important aspect to understanding and helping our world it must be done ethically. Human subjects must willingly volunteer to be participants in the research and their care before, during, and after has to be done ethically at all times. Ethical guidelines are set to protect research volunteers and also to preserve the integrity of the science. While the earnings of health care professionals have been increasing, insurance premiums have also increased. Among hospitals, many are non-profit organizations offering substantial charity care. But for-profit and non-profit hospitals alike must show return on investment to remain viable. Providers must also factor reimbursement policies of payers into their decisions, sometimes even clinical decisions. These groups of stakeholders—patients, providers, payers, and the government—have different goals and different responses to the rising cost of care.

A public health crisis is described as an emerging situation that affects a large population, often spanning a wide geographic area, usually in a very short period of time. For instance, in the very first chapter of his book, On Washing Hands, Dr. Atul Gawande (2007) describes how inexplicably common it is for hospital personnel to not wash their hands and cause the spread of infections. In the United States, it has been estimated that approximately two million health care associated-infections (HAI) occur each year, resulting in 60,000 to 90,000 deaths. The cost is estimated to be at least $17 to $29 billion (Jarvis, 2007).
At a global scale, health crises may be even more difficult to manage owing to several complexities. This is when the infection rates from super-resistant bacteria have increased throughout the world, and humans travel in unprecedented numbers and at a very fast pace. For example, transmitting primarily through the hands of health care workers, the SARS virus—having first appeared in the Guangdong Province of China in late 2002—spread within a few months to over twenty five countries, infecting over 8,000 people and causing 774 deaths, as reported by the Centers for Disease Control and Prevention (CDC). There are several opportunities for such incidents to quickly escalate to crises levels.
Public health emergencies may come from several other sectors as well. In its website, the office of the Assistant Secretary for Preparedness and Response (ASPR) of the US Department of Health and Human Services (HHS) lists four major kinds of emergencies:

  • Oil or chemical spills
  • Biohazard or infectious disease
  • Food safety issues
  • Terrorism or criminal activity

It is very likely that all of these emergencies will require health care emergency managers to plan and prepare for contingencies.
The outcome of how a crisis is managed is largely dependent on the knowhow, experience, and the personality of the manager. No matter how much planning is done and protocol followed, emergency managers need to have strong leadership qualities in order to handle crises. Having a working knowledge of the health care field is also critical in this context. During an emergent health crisis, the manager needs to coordinate between various groups, communicate decisively and clearly in order to avoid panic, and optimize the distribution of resources.
